Customize Form from Action Menu

I am struggling to get a web form to open after customization. I am currently trying to customize the Work Queue form when opened from the Office MES form from the main client. I can open the form and customize it without issue. I can then deploy the form as a Web Access Form. However, I cannot get this form to open when using Web Access. I am also unable to get the customized form to open when accessing the action menu from the Office MES form > Actions > Work Queue. Can anyone offer any help on what I am missing? I can see the forms deployed to the proper folder on the server as well.

When you click on the actions work queue does it bring up the extra Process Calling dialog? Like

If so perhaps you have to add the customization to the menu item that is called in process calling. I’ve never used work queue so it’s just a stab in the dark here.
