"Clocking out" periodically doesn't work but Vantage/user thinks

I've seen this a few times also. It will let the user have multiple active LaborHed records. I have never been able to figure out what causes this, has to be a bug.

We have an issue sporadically that a user will “clock out” and vantage doesn’t give any feedback that it didn’t work but when we come in the next morning, it still has them clocked in.  Has anyone seen this type of issue and if so, what causes it and what might be the solution????  It doesn’t happen all the time, but it’s a problem since we have a points system that penalizes employees if they “forget” to clock out each day.
