Bob:
I've been running Proxy server with a cable modem for over two years.
Never a problem. Dual nic cards, one to the inside and one to the outside
through the modem. Of course, like Dave, I don't keep too much on this box.
Just the bare minimum to run and some other stuff that I could get along
without or rebuild if anything ever happened.

no problem. I have a home build dual nic'ed 300 MHz
machine that runs only proxy server and serves as a software repository.
That way if anyone ever got to this machine they couldn't
hurt much. A restore from backup would only take about 1 hour.

I have never had any trouble with this machine at all as far as anyone
trying to break in.

You should have a fair amount of free disk space to use for caching urls.

We have had a dial link that is connected weeks at a time so it behaves like
a full time connection. MS-Proxy Server has been used to limit outward
access but not as a security tool. We have had a lot of problems with Proxy
Server preventing access to customer B-2-B web sites which run various java
scripts and access specific IP ports so now many users bypass and talk
directly to the gateway. When we install a new T1 line next month it will
be via a SonicWall firewall and I will drop Proxy Server entirely. As for
hacking, not any breaches as far as I know but there have been a few really
odd login fails in the security logs. These indicate that it is probably
possible to get in but these have all been from other customers of the ISP
and mostly likely the result of the IP address changing on every re-dial-in
as opposed to deliberate attempts. The variable IP address helps and our
web and email DNS addresses are elsewhere at remote hosts so this provides
extra protection like Network Address Translation.

Bottom line is I don't like Proxy Server because it causes more problems
than it fixes (for us) and feel a lot better behind a hardware firewall for
the full-time connection. The SonicWall will add NAT, access control and
site (content) filtering as well as a future DMZ for a web server so it has
more features than Proxy Server. I am not familar (yet) with ISA. Just my
experiences and I'm no Proxy Server / NT expert.
